First thing I can think of: The end of the marketplace fight in Raiders of the Lost Ark where Indy shoots the swordsman was improvised by Harrison Ford, who was extremely sick the day they shot the scene & didn't wanna do the scripted swordfight.

One of my favourites is from 'Die Hard'. Hans Gruber has this great look of surprise on his face when John McClane drops him off the building at the end-------evidently the stunt coordinator told Alan Rickman they were going to drop him after a five-count so that he would be ready for the drop when it happened, and then they dropped him on three instead.

On the set of 'Psycho', the fake blood they were originally using during the shower scene wasn't making enough of a visual impact on black-and-white film, so they switched it out for Bosco chocolate syrup. George Romero wound up doing the same thing when he made 'Night of the Living Dead'.

When they're sewing up that dude's arm in Dawn of the Dead they're accidentally actually doing it and he just takes it like a champ.

For the scene in Return of the King where Wormtongue murders Saruman, Christopher Lee taught Peter Jackson the correct sound a person makes when they're stabbed in the back, based on Lee's personal experiences both witnessing & causing such deaths in World War II.

Hey, hey, bump.

Hey, so I have a piece of 'It's a Wonderful Life' trivia that isn't about the film reels falling over.

So, in 'It's a Wonderful Life', Uncle Billy had some unusual pets, and one of them was a crow. We see it several times throughout the film, on Billy's shoulder, or just standing on the counter at the Building & Loan and cawing. Outside of the universe of the film, this crow was a highly-trained bird named Jimmy, who (among other things) would come when he heard his name. So, like, in-between takes, Frank Capra would occasionally be speaking to Jimmy Stewart, either man-to-man or through a megaphone, and it led to so many instances of Jimmy the crow flying in and perching on Stewart's shoulder or Capra's head or something that eventually Capra started calling Stewart 'JS' on set.
